The form is 10.5x6.5 cm, with the image of a young Austrian "originator" of Serbian origin, whose name is MILOŠ SVITAŠ, printed with a cyrillic brown ink on the back of the photograph, with the date of June 1916.
On the chests of the officer is the Austrian Order of Merit.
